I hereby acknowledge that the health of my players is good and they are able to participate in D1 Minnesota events. In the event of injury during the event, I authorize the event staff to secure any medical treatment that they deem necessary. I further agree to waive any liability whatsoever from D1 Minnesota staff on other personnel in the event
